 ALL SCOUTS and ADULT volunteers
that are part of the troop (if you're on
the troop roster, you need to fill this
out - this includes committee
members and scoutmasters).
Part B
 EVERY 3 YEARS
(Physical exam required every 3
years)
 SCOUTS and ADULTS UNDER 40
attending trips over 72 hours in
duration, a resident setting (summer
camp), or a trip of strenuous nature)

Transportation Registration Forms must be current for each driver used to transport scouts
for any purpose. Please submit this form and review it annually for correctness.

Permission Slips are provided for each trip, they must be filled out and signed by both the scout
and a parent/guardian. They are handed out at troop meetings and are also emailed to the troop. If
you are not getting one, contact the Troop Secretary. Forms are to be turned in at the troop
meeting prior to the trip. Dates are listed on the form.

Photo Release for Adults ( Fill out and return to Scoutmaster to allow your photo/first name to
appear on this website and/or other publications.)

Photo Release for Minors ( Fill out and return to Scoutmaster to allow your son's photo/first
name to appear on this website and/or other publications.)

Expense Reimbursement (Fill out and return to the Treasurer whenever you incur a troop related
expense).
