CLASS II
 EVERY 3 YEARS
(Physical exam required every
3 years)
 SCOUTS and ADULTS UNDER 40
attending trips over 72 hours in
duration
CLASS III
 ANNUALLY
(Physical exam required
annually)
 ADULTS OVER 40 attending trips
over 72 hours in duration
Generic Permission Slip ( Use this generic permission slip ONLY if you don't have the specific
one for the outing. Turn in to scoutmaster or assistant scoutmaster )
Photo Release for Adults ( Fill out and return to Scoutmaster to allow your photo/first name to
appear on this website and/or other publications. )
Photo Release for Minors ( Fill out and return to Scoutmaster to allow your son's photo/first
name to appear on this website and/or other publications.)
Expense Reimbursement & Revenue Receipt (Fill out and return to the Treasurer whenever
you incur a troop related expense).
